Coronavirus disease 2019 (COVID-19) vaccinations remain relevant in 2025, especially for high-risk groups. The safety of vaccines available in Germany is similar to other vaccinations. Allergic reactions to COVID-19 vaccines, especially severe anaphylactic reactions, are rare but occur more frequently in women after the first mRNA vaccination. The risk of anaphylaxis due to mRNA vaccines is similar to other vaccines. Polyethylene glycol (PEG) and polysorbate 80 are considered potential triggers. Diagnostic evaluation of allergic reactions using skin tests and basophil activation tests has shown that the majority of reactions are not IgE-mediated. A structured approach to risk assessment enables a safe vaccination strategy for individuals with allergies and helps reduce fears to improve vaccination willingness.
