Cross-sectoral care in oncology is considered key to improving efficiency, quality of care, and patient outcomes. However, health economic analyses paint a sobering picture. Studies of certified oncology centers show that coordinated care can reduce overall costs per patient and improve survival, yet the specific contribution of cross-sectoral integration remains unclear. Despite positive effects on patient satisfaction, the existing economic and organizational evidence is insufficient to justify implementation in routine care. Reductions in duplicate diagnostics or rehospitalizations are often assumed but only sparsely quantified empirically. Major barriers include limited digital interoperability and inadequate reimbursement mechanisms, while investment requirements—particularly for IT infrastructure and interface management—are substantial. Overall, without robust outcome measurement, appropriate incentive structures, and long-term financing, potential benefits cannot be realized. Development of evidence-based governance mechanisms is therefore essential to ensure that cancer care is both fit for the future and economically sustainable.
