<p>Hypertensive disorders of pregnancy, particularly preeclampsia (PE), significantly contribute to maternal and fetal morbidity and mortality. Identifying reliable biomarkers for predicting adverse outcomes in PE is critical for optimizing obstetric care. N Terminal-pro Beta Natriuretic Peptide (NT-pro BNP), a marker for cardiac strain, has shown potential in predicting PE-related complications. To evaluate the role of second-trimester NT-proBNP levels in predicting adverse outcomes in preeclampsia. This prospective observational study included 139 pregnant women at the Himalayan Institute of Medical Sciences, India, from January 2021 to December 2023. Serum NT-proBNP levels were measured in the second trimester using enzyme-linked fluorescent assay. Participants were followed up, and pregnancy outcomes, including delivery timing, mode of delivery, and birth weight, were recorded. Statistical analysis was performed using SPSS version 21, with significance set at <i>p</i> &lt; 0.05. Of the 139 participants, 79 developed PE, with 68 exhibiting elevated NTproBNP levels. Elevated NT-proBNP levels were significantly associated with preterm delivery (<i>p</i> = 0.015) and showed a moderate correlation with caesarean delivery (<i>p</i> = 0.361). NT-proBNP levels demonstrated a strong negative correlation with birth weight (r = −&#xa0;0.383). Among normotensive participants, 13 of 60 displayed NT-proBNP levels above the cutoff of 60 pg/mL. Elevated second-trimester NT-proBNP levels are associated with adverse pregnancy outcomes, including preterm delivery, caesarean section, and low birth weight, in preeclamptic patients. These findings suggest NT-proBNP could serve as a predictive biomarker for identifying high-risk pregnancies and facilitating timely interventions.</p>
