Prevalence of High ABO - Antibody Titers in O blood Group Donors: An Observation from Eastern India

ABO isoagglutinin can cause severe intravascular hemolysis, renal failure and death in transfused patients. They also play a significant role in the outcome of ABO incompatible grafts in both solid organs as well as hematopoietic stem cell transplants. The present study was aimed at identifying prevalence of high titer (IgG/IgM) group O blood donors. 300 blood group O donors were selected by simple randomization. Blood donors’ antibody titration for IgM was done by serial dilution of plasma at 240 C by conventional tube technique. After that, DTT treatment of plasma was carried out and antibody titration for IgG antibodies was done. The median antibody titers in females were greater than those in men in general. A higher median antibody titer was seen in the higher age group for anti-B IgM (64) and anti-A IgG (64). There was a positive correlation between different antibody titer. The mean ABO antibody titers (IgG and IgM) in O group donors in our region are notably high. Therefore, antibody titration might be considered for all O group single donor platelets and prior to group O whole blood transfusion in trauma.
