<p>For more than two decades the standard treatment for patients with resectable locally advanced squamous cell carcinomas of the head and neck region (LA-HNSCC) and pathological high-risk characteristics has been based on surgical resection and adjuvant chemoradiotherapy with cisplatin; however, the limitations of this treatment approach are inestimable: a large proportion of patients (ca. 40–45%) suffer a recurrence, which is often coupled to an unfavorable prognosis. A potential turning point is presented by two recently published phase III studies: KEYNOTE-689, which investigated a perioperative concept with pembrolizumab and NIVOPOSTOP, which follows a purely adjuvant approach with nivolumab. The objective of the present comments is to undertake a critical and comparative assessment of these two groundbreaking studies and their different approaches.</p>
