Trigeminal autonomic cephalalgias comprise a group of primary headache disorders with very severe, strictly unilateral periorbital pain attacks accompanied by autonomic symptoms such as lacrimation and rhinorrhea. There are four defined forms of these headache disorders (cluster headache, paroxysmal hemicrania, SUNA/SUNCT, and hemicrania continua), which can be distinguished mainly in terms of their temporal patterns. Trigeminal neuralgia does not belong to these headache disorders. Evidence-based treatment guidelines have been published, and the majority of sufferers can be effectively treated with pharmaceutical and interventional procedures. The analgesic indomethacin plays a specific role, as it is completely effective in some of these headache disorders. Patients with very severe paroxysmal headaches which are not typical for one’s own medical field should be referred to a neurologist or a pain clinic.
