<p>Although allergies to beta-lactam antibiotics, especially penicillins, are among the most frequently reported drug allergies, the majority of patients can tolerate these antibiotics without difficulty. Having a&#xa0;suspected allergy on record is associated with considerable disadvantages for the patient (e.g. poorer efficacy of alternative drugs). As more extensive allergy tests cannot be performed in the outpatient setting, the patient history-based PEN-FAST score was recently developed. This allows patients with a&#xa0;low risk of a&#xa0;genuine penicillin allergy in particular to be identified quickly and in a&#xa0;resource-efficient manner. The exclusion of a&#xa0;suspected diagnosis of penicillin allergy, known as delabeling, is an important part of rational antibiotic therapy. However, for non-allergy specialists in Germany, there are still no guideline-based recommendations for delabeling.</p>
