Primary Neuroendocrine Tumor of the Seminal Vesicle: A Diagnostic and Therapeutic Challenge

We present a case of an elderly male in his mid-70s who presented to the urology outpatient department with lower urinary tract symptoms (LUTS) and was incidentally detected with a right pelvic mass on evaluation. Contrast-enhanced Computed Tomography (CECT) revealed a solid-cystic right pelvic mass with enhancement. Positron emission tomography (PET) revealed a hypermetabolic soft tissue mass replacing the right seminal vesicle, with no evidence of distant metastasis. Fine-needle aspiration cytology (FNAC) revealed a well differentiated neoplasm. The patient underwent laparoscopic excision of the right pelvic mass and standard pelvic lymph node dissection. Immunohistochemistry confirmed a neuroendocrine neoplasm with positive synaptophysin and chromogranin A staining. On the 6-month follow-up scan, he remained disease-free. This case highlights the challenges faced during the diagnosis and treatment of primary seminal vesicle neuroendocrine tumours.
