Air–Fluid Level in the Scrotum: A Rare Radiographic Sign of Strangulated Inguinoscrotal Hernia in a Child

Strangulated inguinal hernia is a surgical emergency in children and is usually diagnosed clinically. [1] Imaging typically demonstrates features of intestinal obstruction, while visualization of bowel within the scrotum on plain radiographs is uncommon. We report a rare radiographic finding of a distinct air–fluid level within the scrotal sac in a young child presenting with intestinal obstruction and acute scrotal swelling. Emergency inguinal exploration revealed gangrenous small bowel within the hernia sac requiring ileal resection and primary anastomosis. Recognition of this unusual radiographic sign may help clinicians identify complicated inguinoscrotal hernia in children, prompting timely surgical intervention.
