A Rare Case of Massive and Obstructive Colonic Metastasis of Bladder Urothelial Carcinoma

Primary urothelial carcinoma (UC) with glandular differentiation can be diagnostically challenging to distinguish from primary colorectal adenocarcinoma. A 74-year-old with a history of low-grade non-invasive papillary UC of bladder presented with abdominal pain and distension. Contrast-enhanced abdominopelvic CT demonstrated a mass in the ascending colon and an adjacent mesenteric lymph node. [18F]FDG PET/CT demonstrated increased FDG uptake in this colonic lesion and adjacent mesenteric lymph node. The patient underwent total colectomy. Histopathological and immunohistochemical analysis confirmed metastasis of UC in both the colonic mass and the mesenteric lymph node. Herein, we report a rare case of massive, obstructive colonic metastasis from bladder UC.
