Pure Androgen Secreting Adrenal Tumour with Hypertension and Inferior Vena Cava Involvement- A Case Report

Purely androgen-secreting adrenal tumors (PASAT) are extremely rare, comprising a small subset of adrenal neoplasms. Even more uncommon is their presentation with resistant hypertension. We report a case of PASAT with invasion of the inferior vena cava (IVC), highlighting its unique clinical presentation, diagnostic challenges, and management strategies. The patient exhibited signs of androgen excess along with uncontrolled hypertension, necessitating a comprehensive diagnostic approach. Imaging and biochemical assessments confirmed the diagnosis, and surgical intervention was performed to address tumour burden and vascular involvement. This case underscores the importance of early recognition and tailored management of PASAT, given its potential for aggressive behaviour and vascular invasion.
