An Extremely Rare Presentation of Intrasplenic Primary Hepatoid Carcinoma in Ectopic Hepatic Tissue with Normal Liver: A Case Report

This case report presents a highly unusual occurrence of intrasplenic primaryhepatocellular carcinoma (HCC) arising in ectopic hepatic tissue, with a normal liver. A 41-year-old female underwent laparoscopic splenectomy for a large splenic mass. Histopathological evaluation revealed moderately differentiated hepatocellular carcinoma producing alpha-fetoprotein (AFP) within ectopic hepatic tissue in the spleen. Immunohistochemistry confirmed the diagnosis with positive staining for AFP, Glypican-3, and CK19. The liver biopsy showed normal histology, ruling out synchronous hepatic malignancy. This case highlights the rarity of ectopic hepatic tissue carcinogenesis and emphasizes the need for multidisciplinary management, including advanced imaging, immunohistochemistry, and molecular studies.
