Free Style Perforator Propeller Flaps for Coverage of Defects Over the Patella

Soft tissue coverage for defects over the patella poses a reconstructive challenge, traditionally addressed through pedicled gastrocnemius flaps. We present our experience with free-style perforator propeller flaps for reconstruction of small to moderate sized defects over the patella, aiming to demonstrate their safety and versatility when meticulously planned and executed. Our study comprises 10 patients with patellar defects of varied etiology treated with free style perforator flaps ranging from 11 cm x 9 cm to 22 cm x 7 cm in size. 9 flaps survived completely, while only one flap had distal marginal necrosis. By the end of 3 months, all flaps & donor areas were well healed and required no additional secondary procedures. Thus, in properly selected patients with small to moderate sized defects over the patella, free style perforator propeller flaps can be a reliable alternative to muscle, fascio cutaneous and free flaps with distinct advantages and minimal complications.
