<p>A 69-year old male patient with a recurrent right forearm sarcoma who was planned to undergo hyperthermic isolated limb perfusion (HILP) is reported. Axillary artery and vein access was initially obtained and vessels were cannulated, but the venous canula could not be advanced distally due to the presence of competent venous valves. Distal passage of the venous canula was facilitated with the use of a through-and-through guidewire which was advanced in a retrograde fashion from the ipsilateral brachial vein. The procedure was completed uneventfully. Vascular surgeons are increasingly involved in oncologic surgical procedures, with the concept of oncovascular surgery having developed in recent years.</p>
