<p>Hepatic infarction is rare, especially in inflammatory conditions like acute necrotizing pancreatitis. Its rarity is attributed to the dual blood supply to the liver, provided by both the hepatic artery and portal vein. In this case report, we present a case of hepatic infarction as a complication of pancreatitis, detected using contrast-enhanced computed tomography (CECT) of the whole abdomen. The patient had severe acute necrotizing pancreatitis with acute necrotic collections and lumen-occluding thrombus in the right portal vein branches, causing hepatic infarcts in segments VI and VIII of the right lobes of the liver and thrombosis of the splenic vein and superior mesenteric vein. The aim of our case report is to elucidate the imaging characteristics of hepatic infarction, demonstrate the importance of the CECT abdomen in identifying this uncommon complication, and discuss its clinical implications.</p>
