<p>Thoracoscopic sympathectomy is known to be an effective treatment for palmar hyperhidrosis. Compensatory hyperhidrosis can negatively impact patients’ quality of life after surgery. This study aims to investigate the long-term outcomes of thoracoscopic sympathectomy and its complications, such as compensatory hyperhidrosis, and their impact on patient satisfaction when evaluated together. The study was conducted with the approval of the local ethics committee dated 21.11.2024 and numbered 85/7. The study included 158 consecutive patients who underwent T3 + T4 thoracoscopic sympathectomy. Of these, 137 were contacted by phone and their information was obtained. Intraoperative and long-term outcomes were recorded. Based on the answers to the questions, the success rate of the surgery was found to be 89.8%. The rate of compensatory hyperhidrosis development was 85.4%. The mean follow-up period was 55.2&#xa0;months (range, 6–98) and the patient satisfaction rate was 89.8%. 78.1% of patients recommended the procedure to other patients undergoing surgery, while 21.9% did not. Thoracoscopic sympathectomy is used as a definitive treatment for the majority of patients with palmar hyperhidrosis and positively impacts their quality of life. There is ongoing debate regarding the surgical strategy at which thoracic level to reduce compensatory hyperhidrosis, a serious complication. We believe that well-designed, multicenter studies using standardized surgical techniques are needed to improve treatment efficacy and, in particular, to reduce compensatory hyperhidrosis.</p>
