<p>Biliary anatomical variants, such as an aberrant right hepatic duct, are critical considerations during laparoscopic cholecystectomy as they can significantly impact perioperative outcomes. An aberrant right hepatic duct draining into the gallbladder or cystic duct is an important anatomical variation because of considerable at risk to injury during cholecystectomy. We report a case of a 71-year-old male who underwent laparoscopic cholecystectomy for symptomatic gallstone disease, where an aberrant right hepatic duct encountered draining into the cystic duct and successfully preserved. This case underscores the importance of employing the Critical View of Safety approach and meticulous surgical techniques to identify and manage such anatomical variations, thereby minimizing the risk of complications.</p>
