<p>Acute appendicitis is a common surgical emergency requiring timely diagnosis. While computed tomography (CT) is highly sensitive, spectral Doppler ultrasound offers a non-invasive alternative, especially for vulnerable populations such as children and pregnant women. To assess the diagnostic accuracy of spectral Doppler ultrasound parameters, including peak systolic velocity (PSV) and resistive index (RI), in suspected appendicitis. A case series analysis was conducted over eight months at Rama Medical College Hospital and Research Centre, Kanpur. A total of 180 patients with suspected appendicitis were screened, with 61 included in the final analysis. Spectral Doppler was used to measure PSV and RI, and diagnosis were confirmed via histopathology or clinical follow-up. Statistical analysis included t-tests, sensitivity and specificity calculations, and receiver operating characteristic (ROC) analysis. Of 61 patients, 25 were diagnosed with appendicitis. The mean PSV was significantly higher in appendicitis cases (18.9&#xa0;cm/sec) compared to non-appendicitis cases (6.8&#xa0;cm/sec, <i>P</i> &lt; 0.0001). A PSV threshold &gt; 10&#xa0;cm/sec yielded 85.3% sensitivity and 92.5% specificity. RI values &gt; 0.65 showed 64.0% sensitivity and 95.0% specificity. ROC analysis demonstrated high diagnostic accuracy, with an area under the curve of 0.96 for PSV and 0.85 for RI. Spectral Doppler ultrasound improves diagnostic accuracy in appendicitis, particularly for equivocal cases on grayscale imaging, offering a reliable, non-invasive alternative to CT in selected patient populations.</p>
