<p>Perihilar cholangiocarcinoma often presents a poor prognosis, with surgical resection being the primary curative option. Emerging evidence suggests that laparoscopic surgery may offer promising outcomes. This case report details a successful laparoscopic approach for Klatskin tumor type IIIb. A 49-year-old male presented with epigastric pain. Initial laboratory results showed normal levels of CA19-9, bilirubin. Imaging confirmed perihilar cholangiocarcinoma type IIIb. A laparoscopic procedure was performed. The operation lasted 440&#xa0;min, blood loss 300&#xa0;ml with no transfusion, and R0 resection margin was achieved. The patient was discharged after 11&#xa0;days without complications. Laparoscopic surgery is feasible to get radical resection for perihilar cholangiocarcinoma. Further studies are needed to evaluate long-term outcomes and refine its indications.</p>
