A rare case of the cystic duct draining directly into the right hepatic duct is being presented, marking only the second such MRCP (magnetic resonance cholangiopancreatography) reported case in contemporary literature. This unusual biliary anatomy poses significant risks during laparoscopic cholecystectomy, emphasizing the need for heightened surgical vigilance.
