<p>A multi-criteria decision-making framework was developed using the Analytical Hierarchy Process (AHP) to evaluate and prioritize biological drug therapies for psoriasis treatment. The model incorporates six key criteria identified through an extensive literature review and validated by a panel of 15 multidisciplinary experts: therapeutic efficacy, safety and tolerability, route of administration, economic viability, patient compliance, and sustainability of therapeutic effects. Pairwise comparisons conducted by the expert panel assigned the highest relative weights to therapeutic efficacy (0.427–0.502) and safety/tolerability (0.258–0.262), followed by the sustainability of therapeutic effects (0.100–0.133). The AHP model demonstrated strong internal consistency (consistency ratio &lt; 0.1), supporting the reliability of the decision framework. Results indicated that IL-17/23 inhibitors, particularly secukinumab, ranked highest in efficacy, in alignment with real-world clinical outcomes. Sensitivity analysis confirmed the robustness of the model, with therapeutic efficacy consistently emerging as the dominant criterion, even under ± 20% variation in weights. The findings underscore the need to balance high clinical efficacy with patient-centric considerations, such as dosing frequency and affordability, to improve long-term treatment adherence. Biologics offering durable remission, such as ustekinumab, were favored in scenarios prioritizing sustainability, while cost-effective options were highlighted as essential in resource-limited settings.
