Cross-Validation of an Embedded Performance Validity Test in the Brief Visuospatial Memory Test-Revised with Spanish-Speaking Patients Residing in the United States

Despite the growing literature on embedded performance validity tests (PVTs), few studies have examined their use among Spanish-speakers living in the United States. The current study investigated the utility of the Brief Visuospatial Memory Test-Revised (BVMT-R) Recognition Discrimination Index (RD) for detecting performance invalidity in a Spanish-speaking sample. Archival data for 66 Spanish speakers were used, including 25 noncredible participants in litigation (N-PIL), 27 neurological patients, and 14 healthy controls recruited from the community. All participants completed the BVMT-R and Test of Memory Malingering (TOMM). Participants in litigation were deemed noncredible based on failure of at least two of the following performance validity tests: TOMM, Dot Counting Test, Rey 15-item Test, and Reliable Digit Span. BVMT-R RD index differed by subgroup, with significantly worse performance in the N-PIL group, compared to the healthy controls and neurological patients. BVMT-R RD index significantly differentiated noncredible from valid performance across groups (areas under the curve = .79-.90). An RD cut-off of ≤ 2 (56% sensitivity/90% specificity) was most appropriate for clinical and forensic cohorts, while a cut-off of ≤ 4 (76% sensitivity/93% specificity) proved optimal for discerning performance accuracy in healthy controls. Overall, findings provided preliminary support for the utility of the BVMT-R RD index as an effective embedded PVT to identify invalid test performance in diverse samples of Spanish-speakers across forensic and clinical contexts.
