Chancen und Grenzen neuer Formen der Device-basierten Rhythmustherapie

Device-based rhythm treatment is a central therapeutic component for bradycardia and tachycardia arrhythmia. It is a pillar of heart failure treatment alongside drug treatment. It includes pacemakers (PM), implantable cardioverter defibrillators (ICD) and cardiac resynchronization therapy (CRT). The development of leadless pacers (LLP) and extracardiac defibrillators in the form of subcutaneous and extravascular ICDs (S-ICD, EV-ICD) resulted in a reduction of device-associated complications and is being further developed. New physiological stimulation techniques (e.g., His bundle and left bundle branch area pacing, LBBAP) provide a physiological alternative to conventional CRT and can reduce the risk of pacemaker-induced cardiomyopathy. A careful interpretation of the indications and selection of the optimal technology for the individual patient are crucial for successful treatment.
