<p>The prevalence of substance use in North Korea has escalated to epidemic levels. This study employed grounded theory to elucidate the phenomenon of substance use among North Koreans and the myriad influences on this behavior. Through interviews with 12 North Korean defectors and four experts, the central phenomenon emerged as routine encounters with drugs in daily life. This study identified the influence of drug use among North Koreans in three key domains. Firstly, environmental factors, characterized by the anti-human policies of the North Korean regime, lead to deficiencies in basic human needs, thereby facilitating accessibility and availability of drugs. Secondly, social-relational factors play a role in both access to and attitudes toward drug use. Lastly, individual factors encompass psychological vulnerabilities and strengths, social status, economic level, and health status. These findings underscore the unique contextual factors that influence substance use among North Koreans and provide insights for future research.</p>
