<p>Antenatal depression (AD) is a prevalent mental health concern closely associated with adverse birth outcomes, maternal health, and socio-economic burdens. This study aims to systematically evaluate the prevalence of antenatal depression among pregnant women in China. A systematic search was conducted using PubMed, Web of Science, Embase, the Cochrane Library, CNKI, CQVIP, Wan Fang Data, and CBMdisc to identify cross-sectional studies on the prevalence of antenatal depression in China. The search period spanned from the establishment of the databases to July 2023. Literature retrieval, extraction, and quality evaluation were independently cross-compared by two researchers. A total of 31 studies were included, representing 83,173 pregnant women. The pooled prevalence of antenatal depression in China was 22% (95% CI, 17–28%, <i>p </i>&lt; 0.001). The detection rates of antenatal depression in first, second and last trimester were 29% (95% CI, 22–38%, <i>p </i>&lt; 0.001), 24% (95% CI, 16–35%, <i>p </i>&lt; 0.001), and 23% (95% CI, 15–34%, <i>p </i>&lt; 0.001), respectively. Antenatal depression is prevalent among Chinese women, exhibiting varying levels of severity. Appropriate attention should be given to the early screening and intervention of antenatal depression in pregnant women.</p>
