Objectives <p>To evaluate the effect of omega-3 supplementation on heart rate (HR) control and heart rate variability (HRV) in children with inappropriate sinus tachycardia (IST).</p> Methods <p>This prospective randomized controlled trial included 60 children (aged 12–16 y) diagnosed with IST. Participants were randomized into two groups: Group A received omega-3 supplementation for 3 mo, while Group B received standard follow-up without treatment. Baseline and post-treatment HR, HRV parameters [Standard deviation of NN intervals (SDNN), Root mean square of successive differences (RMSSD), pNN50], and clinical outcomes were assessed using 24-h Holter monitoring and validated scales.</p> Results <p>After 3 mo, the omega-3 group showed significant reductions in maximum HR (from 160.5 ± 17.9 to 142.2 ± 15.7 bpm, <i>p</i> = 0.000) and average HR (from 98.9 ± 4.7 to 90.7 ± 5.5 bpm, <i>p</i> = 0.000). HRV indices also improved significantly: SDNN (<i>p</i> = 0.000), RMSD (<i>p</i> = 0.000), and pNN50 (<i>p</i> = 0.000). In contrast, no significant changes were observed in the control group. Clinical improvement was significantly higher in the omega-3 group (63.3% marked improvement) compared to controls (<i>p</i> &lt;0.001). Obesity (<i>p</i> = 0.049) and pre-hypertension (<i>p</i> &lt;0.001) were associated with poor response to omega-3 therapy. No significant differences in anxiety scores were found between groups.</p> Conclusions <p>Omega-3 supplementation significantly improved HR control and HRV in pediatric IST patients. Obesity and elevated blood pressure may limit treatment efficacy.</p>
