Epidemiology and Burden of Rotavirus-Associated Acute Gastroenteritis Among Children Aged Under 5 Years in Gujarat, India, from 2020 to 2023

Rotavirus is a common cause of severe acute gastroenteritis (AGE) in children aged under 5 y, worldwide and in India. In this study, the authors describe the epidemiology and burden of rotavirus AGE (RVAGE) among children aged under 5 y in Gujarat, India, from 2020 to 2023, to determine the prevalence of rotavirus infection and to describe the seasonal and genotype patterns and clinical characteristics of RVAGE.
MethodsA prospective, hospital-based surveillance was conducted at four tertiary-care government hospitals in Gujarat. Children aged under 5 y admitted with AGE were enrolled, and their clinical, demographic, and vaccination data were collected. All stool samples positive for rotavirus on enzyme-linked immunosorbent assay (ELISA) were genotyped using reverse-transcription polymerase chain reaction.
ResultsOf the 1,220 children enrolled, 1,156 (94.8%) had adequate stool samples for ELISA. The overall rotavirus positivity was 17.9% and was highest in children aged 12–23 mo (22.2%). Rotavirus positivity peaked between November and February. Compared with rotavirus-negative participants, children with RVAGE had a significantly higher likelihood of experiencing vomiting (odds ratio [OR]: 1.99, 95% confidence interval [CI]: 1.41–2.81) and severe to very severe diarrhea (OR: 1.91, 95% CI: 1.37–2.67). The most prevalent rotavirus genotypes were G3P[8] (43.6%) and G2P[4] (30.5%).
ConclusionsRVAGE is a substantial burden among children aged under 5 y in Gujarat, India, particularly among infants and toddlers. This study highlights the importance of continued rotavirus surveillance and the requirement of widespread vaccination to reduce morbidity and mortality associated with RVAGE in this population.
