Background <p>Breast cancer (BC) incidence is estimated to achieve over 3-million new cases and one million deaths by 2040. Beyond its physical toll, breast cancer detrimentally affects quality of life, imposing emotional, social, and financial burdens on patients and their families. Statins, commonly prescribed for managing cardiovascular health, have garnered attention for their potential role in breast cancer management. Emerging evidence suggests that statins possess anti-proliferative properties, which could impact BC progression. Understanding the potential benefits of statin therapy in BC patients is crucial for optimizing treatment strategies and improving outcomes.</p> Methods <p>Hazard ratio (HR) with 95% confidence intervals (CIs) were calculated to evaluate the outcomes. The DerSimonian and Laird random-effects model was used for all pooled analyses. Heterogeneity was assessed using the I<sup>2</sup> statistic with corresponding 95% CIs, considering <i>I</i><sup><i>2</i></sup> &gt; <i>25%</i> as indicative of substantial heterogeneity. Statistical analyses were conducted in R (version 4.3.2) using the “meta” package, with a significance level set at <i>P</i> &lt; <i>0.05</i>.</p> Results <p>A total of 31 studies and <i>344,936</i> patients were included, of whom <i>72,784</i> (<i>21.1%</i>) was in the statins group and <i>272,152</i> (<i>78.9%</i>) was in the non-statin users group. Statin therapy was associated with a significantly reduced risks of all-cause mortality (<i>HR 0.8583; 95% CI 0.7980–0.9231; P</i> &lt; <i>0.001; I</i><sup><i>2</i></sup> = <i>81.1%</i>), of BCSM (<i>HR 0.8183; 95% CI 0.7334–0.9132; P</i> &lt; <i>0.001; I</i><sup><i>2</i></sup> = <i>84.4%</i>), and of BC recurrence (<i>HR 0.7911; 95% CI 0.6882–0.9095; P</i> &lt; <i>0.001; I</i><sup><i>2</i></sup> = <i>48.6%</i>). However, the analysis of DFS did not show a statistically significant difference between the groups (<i>HR 0.8415; 95% CI 0.4220–1.6780; P</i> = <i>0.624; I</i><sup><i>2</i></sup> = <i>74.2%</i>).</p> Conclusion <p>Our meta-analysis suggests that statin therapy may confer beneficial effects on clinical outcomes in female breast cancer patients, including reduced all-cause mortality, breast cancer-specific mortality, and recurrence. However, further research is warranted to confirm these findings and elucidate the mechanisms underlying these associations.</p>

Methods

Statin therapy was associated with a significantly reduced risks of all-cause mortality (HR 0.8583; 95% CI 0.7980–0.9231; P < 0.001; I2 = 81.1%), of BCSM (HR 0.8183; 95% CI 0.7334–0.9132; P < 0.001; I2 = 84.4%), and of BC recurrence (HR 0.7911; 95% CI 0.6882–0.9095; P < 0.001; I2 = 48.6%). However, the analysis of DFS did not show a statistically significant difference between the groups (HR 0.8415; 95% CI 0.4220–1.6780; P = 0.624; I2 = 74.2%).

Conclusion

Our meta-analysis suggests that statin therapy may confer beneficial effects on clinical outcomes in female breast cancer patients, including reduced all-cause mortality, breast cancer-specific mortality, and recurrence. However, further research is warranted to confirm these findings and elucidate the mechanisms underlying these associations.
