Effect of Vitamin D3 Supplementation on Treatment Outcomes in Posterior Canal BPPV: A Randomized Controlled Study at a Tertiary Care Center in South India

Benign paroxysmal positional vertigo (BPPV) is commonly managed using canalith repositioning maneuvers (CRM). However, recurrence is a clinically challenging scenario. Emerging research suggests an association between vitamin D3 insufficiency and otoconia instability, which might account for the recurrence. This study aims to evaluate whether short-term vitamin D3 supplementation, in combination with standard BPPV therapy (CRM and Betahistine), may prevent recurrence and improve early symptom relief. Sixty patients with posterior canal BPPV were randomly assigned to two groups. Group A received vitamin D3 supplementation (2000 IU daily for 2 weeks) alongside the Epley maneuver and Betahistine. Group B received the Epley maneuver and Betahistine, but without vitamin D3 supplementation. Participants were followed weekly for 12 weeks to assess recurrence and recovery. Female patients constituted the majority (58.3%). The recurrence rate in Group A was significantly lower, with only 2 patients (6.7%) experiencing a return of symptoms, compared to 7 patients (23.3%) in Group B (p = 0.042). No adverse effects were reported. In Indian adults with posterior canal BPPV, a short course of vitamin D3 offers clinical benefit by reducing recurrence. Given the high background prevalence of Vitamin D3 deficiency, empirical supplementation may improve early recovery and stability.
