<p>Chronic rhinosinusitis with nasal polyposis (CRSwNP) is a persistent inflammatory condition of the nose and paranasal sinuses, often associated with type 2 inflammation and elevated immunoglobulin E (IgE) levels. While polyps and symptoms are commonly assessed, the relationship between serum IgE and symptom severity remains unclear. A cross-sectional observational study was conducted at a tertiary care centre between May 2023 and October 2024 to assess the correlation of S.IgE levels and symptom severity in CRSwNP and to explore the influence of coexisting allergic rhinitis. Fifty patients were assessed for symptom severity using Visual Analogue Scale (VAS), and polyps by endoscopic Meltzer grading. S.IgE was measured from blood samples. The patients were predominantly male (68%) with a mean age of 40.48 ± 12.66 years and experienced symptoms of nasal obstruction (100%), nasal discharge (76%), facial pain/pressure (32%), and smell loss (28%). Most had moderate VAS scores (90%), and elevated S.IgE levels (72%). On statistical analysis, S.IgE and VAS correlated weakly (ρ = 0.084, <i>p</i> = 0.561). However, polyp grade correlated strongly with VAS (ρ = 0.669, <i>p</i> &lt; 0.001) but poorly with S.IgE (ρ = 0.037, <i>p</i> = 0.797). Patients with allergic rhinitis had significantly higher S.IgE levels than those without (median 200 IU/mL (IQR: 162.3, 263.8) vs. 130 IU/mL (IQR: 84, 174.3), Mann Whitney U = 96.5, <i>p</i> = 0.003). Serum IgE levels are frequently elevated in CRSwNP, but do not correlate directly with symptom severity or polyp grade. However, IgE measurement may help identify atopic subgroups who could benefit from targeted biologic therapy.</p>
