MASC-Erading in the Parotid Gland: An Unusual Case of Salivary Secretory Carcinoma

Mammary analogue of secretory carcinoma of salivary glands is a rare salivary gland tumor which has been misdiagnosed for several years under the bracket of salivary acinic cell carcinoma. It has very recently been recognised as a separate entity and henceforth, a greater number of cases have been identified in retrospection. The key identifying feature in this carcinoma is the presence of ETV6-NTRK3 fusion gene, the presence of which has also been established in secretory carcinoma of breast, hence drawing its nomenclature. Patient presented with a swelling in the right preauricular area. Initial fine needle aspiration cytology was suggestive of a parotid neoplasm following which the patient underwent right superficial parotidectomy. Histopathology and immunohistochemistry concluded the diagnosis of secretory carcinoma of parotid gland. Owing to its morphological overlap with other salivary gland neoplasms, secretory carcinoma remains a diagnostic challenge. Careful histopathological and immunohistochemical evaluation is essential for accurate diagnosis and to guide appropriate management.
