Iatrogenic Epidermoid Cyst Following Pediatric Supraglottoplasty: A Case Report

Supraglottoplasty is the first-line surgical intervention for severe laryngomalacia. Reported complications include aspiration, supraglottic stenosis, and granuloma formation. To date, epidermoid cyst formation at the supraglottoplasty site has not been previously reported. We present a 24-month-old female with severe laryngomalacia who underwent CO₂ laser supraglottoplasty with initial symptomatic resolution. At 30 months, she developed recurrent stridor. Endoscopy revealed a cystic lesion at the prior surgical site, which was excised endoscopically. Histopathology confirmed an epidermoid cyst. The patient achieved complete resolution of symptoms without recurrence. This case describes epidermoid cyst formation as a delayed, previously unreported complication of supraglottoplasty. The case emphasizes the importance of considering delayed postoperative sequelae, such as cyst formation, when evaluating recurrent stridor. Histopathological confirmation is essential for diagnosis. Careful surgical technique may minimize risk, and long-term follow-up facilitates early detection and intervention.
