Intraoperative Intrathecal Fluorescein Dye Injection Technique for CSF Rhinorrhoea Localization: A Prospective Study

The endoscopic repair of CSF leak is currently considered the standard of surgical care. Localization of CSF leak presents a significant challenge. There are several methods of localization like CT and MRI Cisternography. This study is to find the effectiveness of intraoperative intrathecal fluorescein dye injection technique complementing radiological imaging to localize the CSF leak. A study on 20 patients diagnosed with CSF rhinorrhoea were taken from a period of June 2022- October 2023. After getting the radiological investigations done, intraoperatively we gave dilute Fluorescein dye intrathecal injection to locate CSF leak site. During the intraoperative period, intrathecal fluorescein dye provided an accurate localization of the defect as well as aided in locating additional leak sites. Two cases had multiple leak sites compared to their radiological findings, which aided to close all the defects. Although pre-operative imaging studies are routinely performed for CSF Rhinorrhoea, the intraoperative intrathecal fluorescein dye injection technique helps to pinpoint the actual defect, facilitating surgical repair.
