Anaplastic Transformation of Diffuse Sclerosing Variant Papillary Thyroid Carcinoma: A Case Report

The diffuse sclerosing variant of papillary thyroid carcinoma (DSVPTC) is a rare and aggressive subtype of differentiated thyroid cancer, often misdiagnosed due to its resemblance to benign thyroid disorders. Anaplastic transformation in DSVPTC is exceptionally rare and typically occurs within the thyroid gland or distant metastatic sites. We report a case of a 74-year-old woman presenting with a large lateral neck mass. Histopathology revealed DSVPTC in the thyroid and anaplastic transformation confined to a cervical lymph node. This case highlights the diagnostic complexity of dedifferentiated thyroid tumours, the potential for nodal-specific transformation, and the enduring value of surgical decision-making in guiding diagnosis and treatment in aggressive thyroid malignancies.
