Supramajor Reconstruction in Head and Neck Palliative Oncosurgery: A Case Series

This is a retrospective case series of 30 patients to study various flaps and their combinations used to cover supramajor defects following palliative oncosurgery.

Patients and MethodsPalliative oncosurgery involves extensive surgical intervention in advanced-stage malignancies, often resulting in supra-major defects. Despite the high likelihood of recurrence and limited prognosis, such procedures are undertaken to improve quality of life. This manuscript explores the evolving role of plastic surgery in addressing complex defects following T4 oral cancer resections. Reconstruction in these cases frequently necessitates the use of multiple flaps, integrating microvascular free flaps with pedicled flaps. This combined approach facilitates single-stage coverage of large defects, minimizes postoperative complications, and enhances both recovery and rehabilitation. The adoption of these advanced reconstructive strategies represents a pivotal advancement in the field of palliative oncoplastic surgery.
ConclusionWith the development of microvascular vascularized tissue transfer and the combination of microvascular free flaps and pedicled flaps, it is now possible to cover huge defects following supramajor oncoplastic surgery using full armamentarium of plastic surgery. It has also changed multistaged reconstructions with local and pedicled flaps into single-staged reconstructions with fewer complications thus it helps in early recovery and rehabilitation of patients.
