<p>The usual practice after Septoplasty is to pack the nose with non-absorbable packing materials. There is now a growing body of evidence on postoperative suturing techniques in Septoplasty in terms of reducing post-operative discomfort. However, nasal physiological parameters have seldom been compared. This study aimed to compare post-operative pain, headache, Olfaction and Mucociliary clearance in 2 groups undergoing nasal packing or transseptal suturing after septoplasty. Olfaction and mucociliary clearance were tested pre-operatively by the Indian Smell Identification Test and Saccharin test. Patients underwent Septoplasty after which either transseptal suturing or nasal packing was done, based on a block randomisation sequence. Outcome measures were pain and headache scores by the Visual analogue scale (1–10) on POD 1 and repeat olfactory scores and mucociliary clearance time on POD 7 and 30. A total of 50 patients were enrolled with a mean age of 27.4 years. There was a significantly lower pain and headache score (p value &lt; 0.01) and shorter duration of hospital stay (p value &lt; 0.001) in the Transseptal suturing group. Both groups showed a significant improvement in olfactory scores by POD 30, which was found to be statistically significant in the Transseptal suturing group (p value = 0.016). Transseptal suturing offers an advantage over Nasal packing not only in reducing post-operative morbidity in terms of pain and headache, but also showed a significant improvement in the physiological functions of the nose compared to the Nasal packing group.</p>
