Paranasal Sinus Involvement Predilection in Allergic Fungal Rhinosinusitis—An Analytical Study

To study anatomical, pathophysiological factors contributing to predilection of paranasal sinus involvement in AFRS in our population. This retrospective hospital-based study was conducted in the Postgraduate Department of Otorhinolaryngology, Head and Neck Surgery, Government SMHS Hospital, Srinagar from January 2023 to June 2025. All patients attending the department, fulfilling the inclusion criteria were included in the study. Inclusion criteria were a diagnosis of AFRS according to the Bent and Kuhn criteria. Exclusion criteria included previous paranasal sinus surgery, and anatomical changes due to facial trauma. Fifteen (60%) out of 25 patients were females. Mean age was 42.8 years. Nasal obstruction was the predominant symptom in 18 patients. 19 patients had unilateral symptoms, with right side (n = 11) more symptomatic than the left (n = 08). Maxillary sinus was involved in 18 (72%) patients followed by involvement of ethmoids sinuses in 13(52%) patients. Sinus opacification and hyperattenuation were seen in 96% of patients. Most common anatomical variation was deviated nasal septum (17 patients), followed by blocked osteomeatal complex (16 patients). Serum IgE was raised in 22 out of 25 patients. Serum eosinophilia was seen in 17 out of 25 patients. Although multiple sinuses can be involved in AFRS, unilateral maxillary sinus involvement is most common pattern. The frequent involvement of maxillary sinus in AFRS is attributed to a combination of anatomical, physiological, and immunological factors.
