<p>To objectively differentiate between conductive and sensorineural hearing loss, determine differential diagnosis in cases of conductive deafness, and evaluate Eustachian tube function. A prospective observational study was conducted among 336 patients of 2–14 years of age presenting to the department of Otorhinolaryngology of a tertiary care hospital in India over a period of 22 months with suspected hearing loss, fulfilling the inclusion and exclusion criteria, to study the findings of impedance audiometry and transient otoacoustic emission (TEOAE) tests. This study included 336 patients, predominantly in primary school age group (6–11 years). 64.29% exhibited signs of middle ear effusion on otoscopy, 13.09% showed retracted tympanic membranes, indicating poor eustachian tube function. Tympanometry revealed Type B curve in 58.33% of patients, indicating middle ear pathology. 53.57% of patients had evidence of underlying middle ear disease and 2.38% showed cochlear pathology on acoustic reflex testing. A statistically significant correlation was found between TEOAE results and acoustic reflex tests (CI-95%, <i>p</i> = 0.000). 208 patients were diagnosed with hearing loss, primarily conductive (96.15%), while 3.85% had sensorineural hearing loss. The findings highlight the high prevalence of middle ear pathology in children, contributing to conductive hearing loss. The study attributed middle ear pathology as the most common cause of hearing loss in school aged children. However, no test of hearing is infallible and a combination of these is required to reach the definite etiology of hearing loss in this age group.</p>
