Nasoseptal Flap-Hadad-Bassagasteguy (HB) Flap Transposition an Innovative Technique for Skull Base Reconstruction

Cerebrospinal fluid (CSF) Leaks in the region of posterior table of the frontal sinus or Sternberg’s canal are challenging situations with respect to reconstructive options due to their location. Hadad Bassagasteguy (HB) flap is the best in reconstructing such defects. We retrospectively reviewed the clinical data of patients who underwent endoscopic endonasal skull base repair for Sternberg’s canal CSF leaks and large posterior table frontal leaks at our centre from 2018 to 2022 using the ipsilateral HB flap transposition technique. Four cases of Sternberg’s canal leak and two cases of frontal sinus CSF leak, with a mean age of 36 years, were included. The aetiology of both frontal leaks was road traffic accidents. The mean follow-up period was 2.5 years in all six cases. All the patients underwent complete mucosalization by 4–6 weeks. The HB flap transposition technique is extremely useful for closing ipsilateral Sternberg’s canal CSF Leaks and posterior table frontal leaks which cannot otherwise be repaired using the regular flap harvesting technique.
