Chronic Rhino Orbital Mucormycosis as a Rare Etiology of Orbital Apex Syndrome in a Patient with Diabetic Ketoacidosis: A Case Report

Chronic or indolent Rhino-orbital mucormycosis is a rare variant of fungal infection affecting nose, paranasal sinuses and orbit, caused by Mucoraceae fungi. Unlike the acute form, which progresses rapidly with high mortality, the chronic type develops slowly over weeks to months and is difficult to diagnose due to its atypical symptoms. We report a case of chronic rhino-orbital mucormycosis in a 62-year-old diabetic male, who experienced bilateral nasal blockage, episodic nosebleeds, loss of smell, worsening drooping of right eyelid, right sided loss of vision, and retro orbital pain, of two months duration, with involvement of right orbital apex on imaging. Diagnosis was confirmed by histopathology.The patient underwent Functional Endoscopic Sinus Surgery (FESS) to remove infected tissue, followed by intravenous Amphotericin B and oral itraconazole for three weeks. The patient had excellent recovery of nasal symptoms and retro orbital pain, improvement in ptosis and is doing well at one year of follow up. However, there was no improvement in vision. High index of suspicion is needed to diagnose this rare entity, which is increasing in non-COVID era.
