<p>There is no available data regarding endotype characterization in CRSwNP in Portugal. The primary objective of this prospective study was to characterize the endotype of medically refractory CRSwNP in a tertiary centre in Portugal. Additionally, the surgical response of our patients was evaluated. Prospective clinical study, approved by the Health Ethics Committee. For endotype characterization, patients who underwent surgery between February 2024 and February 2025 were included. Type 2 endotype was defined by the presence of ≥ 10 tissue eosinophils per high-power field, blood eosinophils ≥ 150 cells/µL or total IgE ≥ 100&#xa0;IU/mL. A total of 25 patients were included for endotype evaluation, with a predominance of male patients (72%) and a mean age of 50.84&#xa0;years (range 22–75). The most common symptoms were nasal obstruction (100%). Eleven patients (44%) had concomitant asthma, and 3 patients (12%) had aspirin-exacerbated respiratory disease (AERD). A predominance of eosinophilic inflammation (type 2 endotype) was present in 23 patients (92%). This study demonstrated a predominance of the type 2 endotype in chronic rhinosinusitis with nasal polyps at our tertiary centre. To our knowledge, this is the first study evaluating the endotype of chronic rhinosinusitis with nasal polyps in Portugal.</p>
