<p>A 50-year-old intellectually disabled adult presented with a 3-year history of unilateral foul-smelling nasal discharge and obstruction. Imaging revealed a large calcified rhinolith eroding the nasal structures. Endoscopic extraction yielded a 5.5&#xa0;cm mass colonized by <i>Actinomyces</i>, highlighting the role of chronic obstruction in fostering bacterial infection. Postoperatively, the patient developed epistaxis necessitating nasal packing and intravenous antibiotics, underscoring the complications of delayed intervention. This case emphasizes the importance of early diagnosis to prevent irreversible structural damage and secondary infections.</p>
