Platelet Rich Fibrin(PRF) and Platelet Rich Plasma (PRP) in Endoscopic Cerebro Spinal Fluid (CSF) Rhinorrhoea Repair: A Novel Alternative to Synthetic Fibrin Sealant

In cases of spontaneous CSF rhinorrhea, the endoscopic repair using Hadad flap is the gold standard treatment. It is augmented with synthetic fibrin sealants to increase the success rates. We propose autologous fibrin sealants for the same with similar results.
Method10 patients with spontaneous CSF rhinorrhea who underwent endoscopic CSF leak repair with Hadad flap was augmented with Platelet rich fibrin (PRF) and plasma (PRP).The post-operative healing was noted and followed up for a period of one and a half year.
ResultThe healing was satisfactory and comparable to synthetic sealants in all patients.
DiscussionMany studies have established the efficiency of synthetic fibrin glue as an adjuvant in the CSF leak repair. However, cost concerns, local stock availability, difficulty in mixing and application and less quantity per pack limit its use. These can be overcome with the autologous fibrin sealants with comparable results.
