<p>Radiation therapy plays a vital role in management of advanced head and neck cancers. However, development of radiation induced sarcomas is a rare sequel, which are distinct secondary malignancies occurring within the irradiated field after several years. Undifferentiated pleomorphic sarcoma is the most common subtype of post radiation sarcomas in head and neck. We present a case of pleomorphic sarcoma tongue, developed after a latency period of 5&#xa0;years in treated squamous cell carcinoma of posterior 1/3rd tongue with chemoradiation. This underscores the importance of distinguishing radiation induced sarcomas from recurrence and the need for long term surveillance in identifying and managing such late sequel of radiation therapy.</p>
