<p>An attempt was made to study clinical and microbiological profiles and to assess hearing status in active cases of chronic suppurative otitis media. A prospective observational study was carried out at a tertiary hospital from January 2023 to June 2024. After obtaining informed written consent from patients with active CSOM, clinical profile of the patient was noted. Hearing status was assessed using pure tone audiometry. Pus from the external auditory canal was sent to the Microbiology department for culture and sensitivity testing. In the present study, CSOM is commonly seen in the young adult population with slight male preponderance. Most patients with CSOM present with ear discharge followed by hearing loss (76%), tinnitus (48%) and vertigo. Mean hearing loss in the right ear was 41.5 + 12.1 dB and mean hearing loss in the left ear was 41.5 + 13.4. 50% CSOM patients were with normal eustachian tube function with Mean Saccharin test time was 22.4 + 10.4&#xa0;min. 95% of cases of CSOM with active disease were culture positive. The most predominant organism isolated from the culture was Pseudomonas aeruginosa followed by Staphylococcus aureus. On analysis of drug susceptibility testing, 96.84% cases were resistant to Ampicillin followed by Ciprofloxacin (58.95%), Trimethoprim/ Sulfamethoxazole (49.47%), Cefazolin (30.53%), Gentamicin (27.37%), Cefepime (26.32%), Cefuroxime (24.21%), Imipenem &amp; Amikacin (1.05% each) while there was no resistance to Piperacillin/ Tazobactam. All CSOM cases were sensitive to Piperacillin/ Tazobactam followed by Amikacin &amp; Imipenem each (98.95%), Cefuroxime (75.79%), Cefepime (73.68%), Gentamicin (72.63%), Cefazolin (69.47%), Trimethoprim/Sulfamethoxazole (50.53%), Ciprofloxacin (41.05%) &amp; Ampicillin (3.16%). </p>
