<p>Melioidosis related parotid abscesses in pediatric cases are rare, especially concerning extra-visceral manifestations. This first documented case of Burkholderia pseudomallei-associated parotid abscess in a Malaysian pediatric patient provides important insights into diagnosis and management. The patient experienced recurrence after a 7-day intravenous ceftazidime course, which is shorter than the 10–14&#xa0;days recommended in Malaysian guidelines, highlighting the importance of treatment duration. Microbiological confirmation was crucial to diagnosis, and extended oral amoxicillin-clavulanate (20&#xa0;mg/kg/dose every 8&#xa0;h for 12&#xa0;weeks) in accordance with Malaysian clinical practice guidelines recommendations ultimately resulted in a cure. This case emphasizes the importance of adequate intensive-phase therapy and extended oral eradication in the management of pediatric melioidosis.</p>

Melioidosis related parotid abscesses in pediatric cases are rare, especially concerning extra-visceral manifestations.
