Evaluating the Combined Efficacy of Zinc, Vitamin A, and Intralesional Steroids in Oral Submucous Fibrosis Management

Oral Submucous Fibrosis (OSMF) is a chronic, progressive, and premalignant condition of the oral cavity, primarily associated with arecoline in areca nuts. Although first reported in 1952, a standardized treatment protocol is still lacking, with various medical and surgical approaches yielding mixed results. This study aims to compare the efficacy of intralesional steroids and hyaluronidase alone versus their combination with oral zinc or vitamin A supplementation in improving clinical outcomes in OSMF. A randomized comparative study was conducted on 75 OSMF patients from November 2019 to November 2020. Patients were divided into three groups: Group A received intralesional triamcinolone and hyaluronidase; Group B received the same plus oral zinc; Group C received the same plus oral vitamin A. Groups B and C showed statistically significant improvement in burning sensation (VAS) at weeks 5 and 6 compared to Group A. Mean VAS improvement was highest in Group C (6.84), followed by Group B (6.52) and Group A (2.88). Interincisal distance improved most in Group C (14.92 mm), then B (12.84 mm), and A (5.16 mm). Most patients in Groups B and C shifted to stage I by the end of treatment. OSMF requires both habit cessation and medical intervention. Supplementing intralesional steroid therapy with zinc or vitamin A offers enhanced outcomes. Their safety, affordability, and ease of use make them ideal adjuncts in OSMF treatment.
