<p>Aim: To review operated cases of septum &amp; turbinate surgery and evaluate the advancement in various surgical techniques in the past 5 decades, rate of complications and results in Indian population. Material &amp; Methods: Retrospective observational study, of 905 cases with septal deviation who underwent surgical management were evaluated. Results: Submucosal resection of septum (SMR) was performed till the early 90s, which evolved via septoplasty using a headlight into endoscopic septal surgery. Earlier, the patients had post operative nasal packing which evolved to intraoperative quilting septal sutures. Both the methods had a similar post-operative result in the long term in this study. Quilting sutures showed significant reduction in early post operative pain and morbidity. The change in trend occurred due to the availability of better surgical equipment, drastic improvement in optics, anesthesia techniques and operating conditions in recent years. The overall improvement in socio economic strata and literacy in the general population has not only improved patient compliance but has also facilitated early visits to the doctor to prevent and treat complications.</p>
