<p>Thyroidectomy wound closure techniques continue to evolve, with tissue adhesive glues emerging as alternatives to traditional staples. This study aimed to compare the efficacy of staples versus adhesive glue for thyroidectomy skin closure. A prospective, randomized, controlled study was conducted at Sri Ramachandra Medical College and Research Institute between February 2023 and April 2024. Fifty patients undergoing thyroidectomy were randomly allocated to either staples (n = 25) or adhesive glue (n = 25) groups. Outcomes were assessed using a visual analogue scale (VAS) for pain on days 1 and 7, the Manchester scar assessment tool at 3&#xa0;months, and patient satisfaction questionnaires. While day 1 pain scores were similar between groups, the glue group showed significantly lower pain scores on day 7 (0.240 ± 0.4359 vs. 0.960 ± 0.8406, <i>p</i> = 0.001). The glue group demonstrated better cosmetic outcomes with lower Manchester scar scores (7.84 ± 0.69 vs. 8.96 ± 1.67, <i>p</i> = 0.003) and higher patient satisfaction scores (9.520 ± 0.5099 vs. 7.720 ± 0.6137, <i>p</i> = 0.001). Tissue adhesive glue for thyroidectomy skin closure reduced postoperative pain, produced superior cosmetic outcomes, and was more patient-satisfying than staples, suggesting it is a viable alternative for thyroidectomy wound closure.</p>
