Study of Anatomical Variations of Carotid Artery and Optic Nerve in Sphenoid Sinus Using Computed Tomography

The sphenoid sinus, one of the paranasal sinuses located within the Sphenoid bone at the skull base, is renowned for its significant anatomical variability and close proximity to critical neurovascular structures (Singh BP et al. Int Otorhinolaryngol Head Neck Surg 5:888–892, 2019). As part of the anterior and middle cranial fossae, it is surrounded by vital elements, including the internal carotid artery and optic nerve. The internal carotid artery, a primary vessel supplying blood to the brain and eyes, is intimately associated with the sphenoid sinus, often creating a groove or track known as the carotid sulcus. Similarly, the optic nerve, essential for vision, passes adjacent to or through the sphenoid sinus, lying between the roots of the sphenoid bone’s lesser wings and connecting to the body of the bone (Nikakhlagh S et al. Jentashapir J Health Res 5(6):888, 2014). This was a hospital-based study conducted at Department of Otorhinolaryngology, PES Hospital, Kuppam, A.P. This study included all the patients undergoing CT scan for paranasal sinuses. CT evaluation revealed anatomical variations: 1. Bulging internal carotid artery (ICA): 79.9%, with 38.2% bilateral. 2. Bulging optic nerve: 30.5%, with 14.6% bilateral. 3. Existence of artery in sinus: 16.9%. 4. Existence of optic nerve in sinus: 11.1%. The study highlights a high prevalence particularly in the younger age group. CT imaging revealed significant anatomical variations in the sphenoid sinus, notably involving the internal carotid artery and optic nerve.
